Amending a Raffle/Bingo License

To AMEND or CANCEL a previously submitted application for a Raffle or Bingo License prepare and submit an Amendment Application Packet.

This packet should consist of:

• FOUR (4) Completed Amendment Applications (each with ORIGINAL signatures and ORIGINAL notary stamp/seal)
• New Sample Raffle Ticket (for OFF-Premise raffles only)
• Letter from the applicant organization asking to amend license

PLEASE NOTE:

Amendment Applications are subject to the same 14 day wait period as an original raffle application.
